Non-dominant, Lesional Frontal Lobe Epilepsy Involving the Prefrontal Cortex

This chapter presents a case of pharmacoresistant non-dominant frontal lobe epilepsy due to focal cortical dysplasia undergoing resective surgery. We discuss the characteristics of interictal and ictal EEG findings in patients with frontal lobe epilepsy. We discuss the importance of utilizing extra-operative functional mapping in identifying negative motor areas in guiding resective surgery. We discuss the surgical considerations in patients with lesional frontal lobe epilepsy, the techniques utilized and their corresponding outcomes, predictors, and complications. We discuss the classification of focal cortical dysplasia, the proposed hypothesis for its pathogenesis, and the histopathological findings within each classification. Lastly, we analyze ictal and post-ictal semiologies associated with the supplementary motor, premotor cortex, and primary motor area.
