Strategic Surgical Approach and Technical Details

The splitting technique, which has been modified and perfected over decades, has largely eliminated previous complications such as vascular bleeding, irregular osteotomy procedures, and unexpected fractures, as well as postoperative malpositioning of the mandibular condyles resulting in postoperative relapse. There is still a residual surgical risk of temporary and sometimes unavoidable permanent an- or hypoesthesia of the lip and chin. This is due to the immediate proximity of the alveolar neurovascular bundle to the osteotomy site, which may result in a lesion of the nerve even with the utmost caution.
