Management of PCL Injuries in the Multiligament Injured Knee

Multiligament knee injuries represent a challenging entity in large part due to the complexities associated with their diagnosis, treatment, and rehabilitation. In the setting of multiligament knee instability with associated posterior cruciate ligament injury, there is further consideration needed in deciding the type of reconstructive procedure to be performed, timing of the intervention relative to the date of injury, and surgical technique, as well a fundamental understanding of the anatomy of the knee. Proper diagnosis of pathology and execution of reconstructive procedures in a safe and effective manner is paramount to restoring ligament integrity and reducing the risk of complications and progression of osteoarthritic changes in the knee. Although a relative paucity of high-quality literature currently exists, subjective outcomes following surgical reconstruction are generally favorable with expected returns to higher-level activities between 9 to 12 months postoperatively.
