Gynecologic and Urologic Problems in Older Women

Women 75 years or older are one of the most rapidly growing age groups. Thus, there is an increasing interest in the field of “geriatric gynecology.” Healthcare providers are expected to become adept at evaluation and management of common gynecologic and urologic problems among older women. The goals of care for a highly functional older woman are essentially similar to those for a younger woman. However, the focus of care could vastly differ in more frail older women with multiple medical comorbidities and diminished level of functionality. The key components of geriatric gynecology are to understand the impact of aging on the physiologic processes in the female pelvic organs and unique aspects of gynecologic care specific to older women, as well as to set individualized treatment goals and expectations, to promote quality of life, and to maintain functional independence. Collaborative efforts should be emphasized to coordinate care and optimize communications among the patient, caretaker, primary care provider, specialist, and other healthcare teams involved in providing care for older woman.
