The Geriatrics Approach to Care

The 5Ms are: what Matters, Medication, Mentation, Mobility, and Multimorbidity. It is a mnemonic to quickly express what value geriatrics and related specialties bring to the care of older adults. It is a way to explain what we do. A related framework evolved in parallel: the 4Ms, which are what Matters, Medication, Mentation, Mobility. The 4Ms underpin the Age-Friendly Health System movement. The straightforward simplicity of the 4Ms or 5Ms, whichever is chosen, has allowed it to catch fire across the country and driven the success of the Age-Friendly Health System movement. This chapter will review each domain and then provide a case of the development of an Age-Friendly Health System.
