摘要

Transition of care (ToC) processes are complex, pivotal events that render patients vulnerable to an array of medical errors. Patients with venous thromboembolism (VTE) are especially prone to adverse events, owing to the high-risk nature of both the disease itself and its treatment. Anticoagulants are commonly implicated in adverse events, which further complicates discharge processes for this population. Multiple studies investigating enhanced ToC programs for patients with VTE or those discharged on anticoagulation therapy have been conducted to determine if such programs have meaningful impacts on reducing re-admissions and complications related to VTE and anticoagulation therapy.
