Hip Chondral Lesions

Hip cartilage injuries represent a significant clinical challenge with implications for both young athletes and the aging population. This chapter provides an overview of the pathogenesis, diagnostic modalities, and evolving treatment strategies for hip cartilage injuries. The hip joint’s intricate anatomy and biomechanics make it susceptible to various forms of cartilage damage, including traumatic injuries, degenerative changes, and inflammatory conditions. This review explores the etiological factors contributing to hip cartilage injuries, ranging from acute to chronic conditions including femoroacetabular impingement (FAI). Accurate diagnosis is crucial for effective management. This chapter discusses the diverse array of imaging techniques, including magnetic resonance imaging (MRI), arthroscopy, and emerging technologies like magnetic resonance arthrography (MRA), which aid in identifying the extent and location of cartilage lesions. Treatment strategies for hip cartilage injuries are multifaceted and continue to evolve. Traditional approaches, such as conservative management and physical therapy, are complemented by surgical interventions, including arthroscopic debridement, microfracture, autologous chondrocyte implantation, and emerging regenerative therapies. This review details the current evidence supporting these interventions, emphasizing the importance of individualized treatment plans based on the patient’s age, activity level, and the specific characteristics of the cartilage lesion. Additionally, advancements in tissue engineering and regenerative medicine are discussed as promising avenues for future interventions, aiming to restore the native hip cartilage and improve long-term outcomes. Despite these promising modalities, there remains a clear need for further research to enhance our understanding of hip cartilage physiology, refine diagnostic tools, and optimize treatment algorithms for these challenging injuries.
