Pectoralis major tendon ruptures represent a relatively uncommon injury that predominantly affects males aged 20–40, especially those involved in weight-lifting activities and occasionally contact sports. Characterized by a specific eccentric mechanism of injury, these ruptures allow for a somewhat straightforward diagnostic process when an in-depth understanding of the relevant anatomy and biomechanics is paired with the use of imaging techniques. This chapter aims to provide a comprehensive analysis of pectoralis major tendon ruptures, delving into every facet of the condition in the context of the latest literature. Given the relatively low prevalence of the injury, the bulk of the available evidence stems from case series, supplemented by a significant body of biomechanical and anatomical studies. Through this examination, this chapter seeks to illuminate the nuances of this condition and aid in the decision-making process, reflecting the state-of-the-art understanding of pectoralis major tendon injuries within the field of sports medicine.
