Peroneal Tendon Subluxation

Ankle injuries are a common reason for athletes to seek medical attention, both in emergency rooms and orthopedic clinics. Peroneal tendon subluxation shares similar mechanisms with lateral ankle ligament sprains, making it essential to maintain a high index of suspicion to prevent misdiagnosis. Clinical evaluation, including a careful physical examination, plays a crucial role in early detection. Patients typically report retro-malleolar discomfort, snapping sensations, and chronic ankle instability, particularly on uneven ground. Dynamic ultrasound and magnetic resonance imaging (MRI) are valuable tools for visualizing peroneal tendons within their groove and facilitating an accurate diagnosis. In acute cases involving low-demand athletes, conservative treatments may be considered; however, it is important to note that these approaches often have a high failure rate. Surgery becomes the preferred option for competitive athletes and individuals with chronic injuries, yielding favorable outcomes. Following surgery, patients should undergo a structured rehabilitation protocol, typically enabling their return to sports within 3–5 months post-procedure.
