Patellar Tendinopathy

Patellar tendinopathy has been historically associated with jumping sports. The main pathophysiologic phenomenon of patellar tendinopathy is tendinosis, which is a degenerative condition rather than an inflammatory disorder. Nonsurgical treatment primarily consists of eccentric exercises, which often have good results. For non-responding patients, other nonsurgical options may be attempted, with PRP injections having the best evidence available. For cases refractory to nonsurgical treatment, surgery is indicated and can be performed open or arthroscopically. The two methods are comparable, but arthroscopic treatment results in a faster return to sports.
