Hip Instability

Hip microinstability is an increasingly recognized cause of nonarthritic hip pain, characterized by excessive joint movement that leads to pain and dysfunction. This chapter reviews the pathophysiology, diagnosis, and management strategies for hip microinstability. The condition arises due to a combination of structural, ligamentous, and muscular factors, with risk factors including hip dysplasia, connective tissue disorders, trauma, and prior surgical interventions. Diagnosis is challenging, as no single test confirms microinstability, and it often requires a combination of clinical examination, imaging, and intraoperative findings. Nonoperative management focuses on rehabilitation and muscle strengthening, while surgical interventions include labral repair, capsular plication, and reconstruction in cases of severe instability. With growing awareness and refinement of surgical techniques, patient outcomes have improved. However, further research is needed to establish long-term management protocols and optimal treatment strategies for different patient populations.
