Ankle ligament injuries represent one of the most common musculoskeletal conditions, particularly in athletes and active individuals, with lateral ligament sprains predominating due to inversion mechanisms. This chapter offers a detailed exploration of imaging in the evaluation of ankle ligament injuries for radiologists. It begins with a review of ankle ligament anatomy, focusing on the lateral complex (anterior talofibular ligament [ATFL], calcaneofibular ligament [CFL], posterior talofibular ligament [PTFL]), medial deltoid ligament, and syndesmotic ligaments. Radiographic findings and appearance of ligament injury on advanced imaging modalities will be presented as well as sequelae and associated injuries that can cause persistent pain.
