Paraphilic Disorders

摘要

This chapter presents information on the diagnostic criteria for each of the paraphilic disorders in the paraphilic disorders chapter of the International Classification of Diseases, Eleventh Edition (ICD-11) published by the World Health Organization and the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition, Text Revision (DSM-5-TR) published by the American Psychiatric Association and information on the epidemiology, development and course, and clinical and diagnostic features of each of these disorders. Etiological theories and major aspects of treatment and a clinical vignette and references are also included. The diagnostic criteria for ICD-11 and DSM-5-TR are compared, as well as reasons for their differences. Associated features common to various paraphilic disorders and assessment issues common to paraphilic disorders in the forensic system are discussed. Differential diagnosis, social, environmental, and cultural features are included. Treatment goals, somatic treatments, psychosocial treatments, combined treatments, treatment-refractory patients, special factors influencing treatment, and issues in the clinician-patient relationship are presented. The long-term successful treatment of a patient with pedophilic disorder is included.
