Incidental Small Tectal Arteriovenous Malformation: Stereotactic Radiosurgery Treatment with Complete Obliteration of the AVM and Excellent Clinical Outcome

The patient, who was 54 years old at the time of initial consultation, presented to his primary care physician with recurrent headaches without focal neurologic deficit. With the exception of obesity, there were no known health problems. After discarding intracranial bleeding, magnetic resonance imaging/angiography (MRI/MRA) revealed evidence of a right-sided infratentorial arteriovenous shunt. Subsequent digital subtraction angiography (DSA) confirmed a small arteriovenous malformation (AVM) in the mesencephalic tectum. The AVM was supplied by the right superior cerebellar artery (SCA) and drained via the vein of Galen (VoG). The lesion was treated with stereotactic radiosurgery (SRS) using a linear accelerator (LINAC), delivering a total radiation dose of 16 Gy in one session. MRI/MRA and DSA 37 months later confirmed occlusion of the arteriovenous shunt. MRI excluded radiogenic damage to the surrounding brainstem tissue, and the treatment was clinically tolerated without neurological complications. This chapter focuses on the management of tectal AVMs through SRS.
