The Global Burden of Disease, Injuries, and Risk Factors Study published in the Lancet in 2024 for the period 2010 to 2021 identified anxiety and depressive disorders as the most prevalent and among the leading 25 causes of morbidity according to age-standardized, disability-adjusted life-years. Mental illness and distress are associated with premature mortality and an increased risk of suicide and likelihood of physical comorbidities. Despite the availability of antidepressant treatments for decades, remission rates of depression remain low and relapse rates high. Only approximately 15% of patients receive minimally adequate treatment. This introductory chapter will provide an overview of the global situation and specifics with ketamine treatment for severe depressive disorders. Obstacles to implement ketamine therapy more widely will be described and steps that should be prioritized for implementation will be presented.
