Cardiac transplant patient home medications (8 years after cardiac transplantation): tacrolimus, prednisone, mycophenolate mofetil, valganciclovir, TMP-SMX. For transplants, the risk for rejection is the highest within the first 3–6 months after transplantation and then within the first year. Biopsy of the transplant in the first year after cardiac transplantation is needed if concerns for rejection. If >1 year after transplantation, patients can develop allograft vasculopathy and thus will need coronary angiography if concerning symptoms.
