Symptomatic acid reflux after 4 weeks of treatment with over-the-counter care with H2 blocker and calcium carbonate, the next step is the addition of a proton pump inhibitor (PPI). EGD is necessary if worrisome symptoms (bleeding, weight loss, dysphagia, anorexia, odynophagia, vomiting, signs of systemic illness) or not responding to twice daily PPI after 4 weeks.
