This chapter deals with the main radiological features of lung cancers that affect the elderly population—adenocarcinoma with the recent histological classification, squamous cell carcinoma, large cell lung cancer, and SCLC. We also consider the follow-up of pulmonary nodules, particularly with relation to the age of the patients, and preoperative staging according to the eighth edition of the TNM Staging Manual. The role of imaging in the evaluation of the effects of radiation and chemotherapy is described, both in regard to cancer follow-up and to the side effects of therapy. Finally, the major radiologic features of pulmonary metastasis from extrathoracic tumors are described, and of benign neoplasms, both likely findings in the elderly.
