They can significantly impact an athlete’s performance and career, resulting in time away from the sport and delayed return-to-play times. This chapter delves into the common patterns of shoulder injuries encountered in athletes engaged in various sports based on commonly involved mechanisms, provides a structured approach to understanding the diverse range of shoulder injuries, with a focus on the imaging features. This chapter covers key conditions such as labral injuries, shoulder dislocations, acromioclavicular joint injuries, and overuse syndromes. Osseous injuries, shoulder instability and rotator cuff injuries, as well as neurological injuries of the brachial plexus and chronic degenerative injuries have been excluded from the discussion as these are explored in greater detail in their separate respective chapters. Imaging plays a pivotal role in accurate and timely diagnosis of these conditions, which is paramount in guiding effective management. Radiographs, magnetic resonance imaging (MRI), and ultrasound are widely used and in in the assessment the injuries based on the authors’ experiences. Computed tomography (CT) is also a useful adjunct in characterising the extent of osseous involvement. Discussion around common injuries and real-world examples of annotated cases, as well as detailed illustrations, are provided to help the readers gain insights into the characteristic imaging findings associated with each condition. It is crucial that radiologists involved in the diagnosis of these conditions, as well as orthopaedic surgeons, sports physicians, and allied health professionals involved in their care have a sound understanding of the injury patterns and common imaging findings to improve patient outcomes and support athletes in their return to optimal performance.
