Pediatric Low-Grade Gliomas: “Ta Panta Rhei”

The phrase «Τα πάντα ῥεῖ» (Ta panta rhei) is attributed to the Greek Presocratic Philosopher Heraclitus (544–484 b.C) and means “everything flows”. Low-grade gliomas in children encompass a diverse group of tumors showing unique characteristics from adults. Recent advances in diagnosis and imaging, molecular profiling, pathology, and surgical techniques have improved our understanding of underlying somatic and genetic alterations and the characterization of distinct subgroups. These developments are anticipated to enhance progression-free and overall survival while improving the quality of survival. Thus, emphasis is given to reducing toxicity and long-term effects, including neurocognitive deficits, and introducing additional treatment approaches, such as targeted therapy. In this rapidly evolving era for pediatric low-grade gliomas, “everything changes, everything flows”. The current review is an updated synopsis of the recent advances regarding diagnosis, molecular histopathology, surgical approach and treatment modalities, and challenging issues correlated with such tumors.
