Diabetes mellitus (DM) has reached epidemic proportions worldwide, with an escalating prevalence that parallels the rise in obesity and sedentary lifestyles. At the same time, cancer remains a leading cause of morbidity and mortality worldwide. There is increasing evidence of a bidirectional relationship between DM and cancer, with DM patients having an increased susceptibility to various malignancies, and cancer itself being a risk factor for the development and exacerbation of DM. Despite the growing recognition of this complex interplay, the underlying mechanisms linking DM and carcinogenesis remain incompletely understood. Epidemiological studies have consistently shown an increased risk of several cancers in people with DM, including pancreatic, colorectal, breast, and liver cancer. Mechanistic studies have elucidated several pathways by which DM contributes to carcinogenesis, including chronic inflammation, hyperinsulinaemia, insulin-like growth factor signalling, and dysregulated glucose metabolism. Antidiabetic drugs, lifestyle factors, and genetic predisposition have been shown to modulate cancer risk in diabetic populations. Clinical studies have highlighted the importance of tailored screening strategies, early detection, and integrated management approaches for patients with both DM and cancer susceptibility. This interdisciplinary investigation highlights the importance of collaborative efforts between clinicians, researchers, and public health practitioners to address the complex interplay between DM and carcinogenesis, paving the way for personalised interventions and improved patient outcomes. Future research should focus on further elucidation of the molecular mechanisms linking DM and carcinogenesis and evaluation of the efficacy of targeted interventions to reduce cancer risk and improve prognosis in diabetic patients.
