Cancer-related stroke is a subtype of stroke including both cancer as a direct cause of stroke through several mechanisms and incidental diagnosis of cancer after stroke. Moreover, already-known cancer and occult cancer are considered in this category. The global increase of stroke risk in patients with active cancer has been confirmed by several studies as well as the occurrence of stroke as the first manifestation of cancer. The diagnostic pathway of arterial thrombosis, including stroke, is much less defined than the pathway of unprovoked venous thromboembolism and the therapeutic strategy lacks evidence. In this chapter the main epidemiological and pathophysiological issues of cancer-related stroke are discussed, followed by clinical, neuroradiological, and therapeutic challenges according to the most recent literature.
