The Role of Inflammatory Cells in Skin Cancer Genesis and Possible Therapeutic Implications

Cutaneous malignant and premalignant lesions, comprising actinic keratoses, basal cell carcinoma, squamous cell carcinoma and melanoma, represent a significant global health concern. The intricate interplay between inflammatory and neoplastic processes has garnered considerable attention in understanding and treating these malignancies. This comprehensive overview examines the pivotal role of inflammatory cells in skin cancer initiation, progression, and immune evasion. We delve into the complex relationship between chronic inflammation and the development of cutaneous malignancies, as well as the involvement of immune surveillance mechanisms in suppressing their progression. In addition, we explore the distinct patterns of immune infiltration and the significance of tumor-infiltrating lymphocytes (TILs) in tumor progression and regression. The emergence of immune checkpoint inhibitors has revolutionized the management of advanced melanoma, and their efficacy and mechanisms of action are described in detail. Furthermore, we summarize the evolving landscape of immune-mediated therapies across the spectrum of skin cancers. This chapter provides insights into the role of inflammatory cells in skin cancer development and highlights the potential for immunotherapeutic interventions to improve patient outcomes.
