Psychological Distress in Patients with Head and Neck Cancer: Exploring the Biological Connections

This chapter explores the complex nature of head and neck cancer (HNC), recognizing it as a challenging condition for patient treatment and the effective management of psychological distress. Psychological distress significantly impacts the quality of life for HNC individuals, with its influence extending across various physiological and pathological pathways. Examining the psychological dimensions involves factors such as physical condition, body image, suicide, social support, history of mental disorders, and challenges posed by the COVID-19 pandemic. Simultaneously, this chapter investigates interconnected biological and genetic aspects, including the hypothalamus-pituitary-adrenal (HPA) axis, inflammation, and the serotonin (5-HT) pathways. An analysis of HNC patient data reveals that psychological distress affects 60% of individuals, leading to the loss of functional capacities like swallowing and speaking. It contributes to fatigue, pain, disruptions in body image, suicidal ideation, reduced social support, and issues related to alcohol and tobacco addiction, alongside a history of mental disorders. Dysregulation of the HPA axis, elevated cortisol levels, chronic inflammation, and abnormalities in the 5-HT pathway serve as prognostic indicators for unfavorable outcomes, intricately linked to heightened manifestations of sickness behaviors associated with psychological distress. Exploring genetic variations suggests potential alterations in responses to stress, with implications for tumor outcomes. Insights into HNC molecular pathways and genetic modulation show promise for targeted therapies, offering paths to enhance the overall quality of life for patients with HNC.
