Breast Cancer: The Fight for Survival Is Won: What Is the Evidence for Preserving Fertility?

Making decisions on fertility preservation while receiving cancer treatment is a difficult and multifaceted task. With an emphasis on breast cancer patients, this chapter examines several choices and factors for fertility preservation. The patient’s age, the cancer’s severity and stage, the need for prompt treatment, her marital status, and her desire for future pregnancies are important considerations when choosing a fertility preservation strategy. The use of gonadotropin-releasing hormone (GnRH) agonists and the cryopreservation of ovarian tissue, embryos, and eggs are among the fertility preservation techniques that are currently available. Every technique is assessed in light of how well it fits various patient profiles and medical scenarios. The explanation of controlled ovarian stimulation (COS) protocols emphasizes the role that hormone regulation plays in the ovarian stimulation process, as well as the use of GnRH agonists or antagonists. There is also discussion of the mechanism of COS and possible issues with embryonic development and chromosomal disorders. The chapter explores new epidemiological data on the preservation of fertility in breast cancer patients, highlighting the efficacy and safety of ovarian stimulation treatments. The success rates and results of studies on cryopreservation techniques—such as oocyte and embryo cryopreservation—are analyzed and discussed. The use of in-vitro maturation (IVM) as an alternative to traditional ovarian stimulation is also covered in the chapter, with a focus on breast cancer patients. Examined are the possible advantages and restrictions of IVM, as well as the use of it in conjunction with ovarian tissue oocyte retrieval. In summary, decisions for fertility preservation made by individuals with breast cancer must take into account a number of variables and available options. The field of oncofertility is constantly changing, which highlights the importance of oncologists and fertility specialists working together to make decisions that take into consideration the particular needs and preferences of each patient. To enhance results for cancer survivors who desire fertility preservation, more research is required to expand and improve the current methods of fertility preservation.
