Improved FMEA Assessment of the Occurrence Probability for Failure Modes or Their Causes

Abstract
In failure modes and effects analysis (FMEA), the method employed to assess the likelihood of occurrence of a specific failure mode is considered. Its strengths and weaknesses are noted. A new approach permits unique determination of a numerical score for the appearance of a failure mode on the basis of relevant historical data. This approach takes account of several measures for fault prevention. As an example, the proposed approach is used in assessing the probability of occurrence for a specific failure mode in FMEA.
