Oncology and Fertility Outcomes for Patients with Stage IC Adult Granulosa Cell Tumor of the Ovary

This study aimed to evaluate the efficacy and safety of fertility-sparing surgery (FSS) for patients with stage IC adult granulosa cell tumors (AGCTs) of the ovary.
MethodsA retrospective study was conducted at Peking Union Medical College Hospital, covering data from 1 February 1986 to 28 April 2024, focusing on women with a diagnosis of stage IC AGCTs. The patients were divided into two groups: those who underwent FSS and those who received radical surgery (RS). The primary endpoints were survival and fertility outcomes.
ResultsThe study included 84 patients who had stage IC AGCTs (55 patients in the FSS group and 29 patients in the RS group). The median follow-up period was 130 months (range, 9–425 months) in the FSS group and 126 months (range, 9–381 months) in the RS group. The recurrence rates were 61.8% in the FSS group and 37.9% in the RS group. Multivariate analysis identified that the application of FSS was significantly associated with decreased disease-free survival (DFS), whereas older age was an independent risk factor for inferior overall survival (OS). In the FSS group, cystectomy was significantly associated with an increased risk of recurrence related to DFS. Additionally, among the patients in the FSS group, 12 conceived, and 11 successfully delivered a total of 13 children.
ConclusionsFertility-sparing surgery should be pursued cautiously, with careful patient selection and comprehensive counseling, in view of the substantial risk for recurrence and the limitations of the current evidence.
