<p>Cochlear implantation (CI) is a widely accepted surgical intervention for individuals with profound sensorineural hearing loss. While the procedure offers significant benefits, postoperative infections remain a serious complication that can impact both the short- and long-term success of the implant. This systematic review aims to evaluate the incidence, risk factors, prevention strategies, diagnostic approaches, and management techniques related to postoperative infections in CI patients. A comprehensive analysis of available literature reveals that infections can occur in both early and late phases post-surgery, with common pathogens including <i>Staphylococcus aureus</i> and <i>Pseudomonas aeruginosa</i>. Key risk factors for infection include patient comorbidities, surgical technique, and the presence of foreign bodies. Prevention strategies such as the use of prophylactic antibiotics, proper sterile techniques, and tailored postoperative care are essential to minimizing infection rates. Early detection through clinical signs and microbiological cultures is crucial for effective management, which often involves antibiotic therapy and, in severe cases, surgical intervention or explantation. Furthermore, the emergence of biofilm formation on implants complicates treatment, necessitating advanced diagnostic and therapeutic approaches. This review emphasizes the need for personalized care plans and multidisciplinary collaboration to improve patient outcomes. By synthesizing current evidence, the study provides valuable insights for clinicians and researchers working to enhance the safety and efficacy of cochlear implantation.</p>
