Background <p>Hepatitis B virus (HBV) infection is a common cause of chronic liver diseases. Interferon gamma (IFN-γ) is a proinflammatory cytokine that plays a crucial role in the immune response to various viral infections. Polymorphisms in IFN-γ may affect the progression and severity of HBV infection. This study aimed to examine the impact of the IFN-γ (rs1861494) gene polymorphism on the clinical outcomes of chronic HBV infection in Egyptian patients.</p> Patients and methods <p>The study included 120 HBV-infected patients divided into three groups: inactive (chronic HBV infection), active (chronic hepatitis B), and cirrhotic patients with no evidence of liver tumors. In addition, 40 healthy individuals were included as controls. After routine examination, the rs1861494 polymorphism of the IFN-γ gene was analyzed using real-time PCR.</p> Results <p>Elevated frequencies of the T allele and TT genotype were observed in patients with cirrhosis. The recessive model (TT versus CC + CT) indicated a significantly increased risk of fibrosis progression compared to the control, with an odds ratio (ORs) of 3.14 (95% <i>CI</i> = 1.12–8.82; <i>p</i> = 0.026). Fibroscan values showed significant increases in cirrhotic patients and the active group with the TT genotype compared to CC + CT cirrhotic patients (<i>p</i> = 0.014 and 0.029, respectively).</p> Conclusions <p>The TT genotype of the IFN-γ (rs1861494) polymorphism could be a risk factor for progression to cirrhosis. Chronic HBV patients with the TT genotype should be closely monitored for early detection of cirrhosis and management of additional factors that may contribute to its progression.</p>
