Arterio-biliary fistula is a rare cause of haemobilia, most often iatrogenic but occasionally resulting from blunt abdominal trauma. Early diagnosis is essential due to its potentially life-threatening presentation.
Case presentationA 24-year-old female presented with abdominal pain and melena two weeks after blunt abdominal trauma. CECT revealed pseudoaneurysms in the right hepatic artery communicating with the right hepatic duct, confirming an arterio-biliary fistula.
DiscussionAngiography confirmed the diagnosis and guided endovascular glue embolization, achieving complete occlusion. The patient’s haemoglobin stabilized post-procedure, with no recurrence of bleeding.
ConclusionPost-traumatic arterio-biliary fistula is an uncommon but serious condition. CT angiography aids in early diagnosis, and endovascular embolization serves as a safe, minimally invasive, and effective treatment, avoiding the need for surgical intervention.
