<p>It is rare for a patient with a supratentorial stroke to result in hemiparesis on the same side of the body. This unusual phenomenon has been documented previously, and various underlying reasons can exist for it. However, its impact on management and medical decision-making for urgent stroke patients has not been explored. In this case, we share our encounter with a 66-year-old male patient who suffered a stroke with an ipsilateral weakness of the same side, a case that is not only rare but also significant in advancing our understanding of stroke management. We discuss the various pathological and anatomical mechanisms possible that might explain this occurrence and the complex decision-making process in his management.</p>
