Detection of arterial thrombosis in acute ischemic stroke using susceptibility-weighted imaging (SWI) compared to the time of flight MR angiography

Acute ischemic stroke leads to serious damage to brain tissue. Serious consequences are noted in patients according to the affected lobe of the brain. Thus, assessing thrombus location and extent is mandatory for diagnosis and to guide the therapeutic management accurately.
Aim of the workWe aimed to compare susceptibility-weighted imaging to TOF MRA in the evaluation and detection of arterial thrombosis in acute ischemic stroke.
ResultsPerfect Kappa agreement (95% CI) (1.000) was noted in the detection of thrombus, in different segments of the Middle cerebral artery (MCA), anterior cerebral artery (ACA), and posterior cerebral artery (PCA), but the substantial agreement was noted (0.787) in evaluated internal carotid artery (ICA). SWI visualized arterial thrombus in MCA with a high sensitivity of 94.3%, specificity of 80.0%, and accuracy of 92.5%. As regards ICA: it revealed 66.70% sensitivity, 100% specificity, and 97.5% accuracy. While sensitivity, specificity, and accuracy were 100% for ACA and PCA.
ConclusionsSWI revealed adequate sensitivity, specificity, and accuracy when evaluating arterial thrombosis compared to MRA in acute ischemic stroke. The 'susceptibility vessel sign' had a high sensitivity in visualization of the site and the extension of acute vascular thrombosis of the main cranial arteries. Furthermore, it can assess the thrombus length beyond the ability of TOF MRA.
