Background <p>Stroke is a major global cause of disability and death, influenced by both environmental and genetic factors. The adiponectin gene (<i>ADIPOQ</i>) polymorphisms, including rs2241767, have been associated with stroke susceptibility. However, evidence regarding the relationship between rs2241767 and ischemic stroke (IS) risk in the Iranian population, particularly in Eastern Iran, remains scarce. This study uniquely examines the association of the rs2241767 variant with IS risk in the Eastern Iranian population.</p> Methods <p>This case-control study included 241 IS patients diagnosed at Birjand Neurology Center and 243 matched controls. Genomic DNA was extracted from peripheral blood samples, and the genotyping of rs2241767 was performed using Tetra ARMS-PCR. Stroke severity and functional outcome were assessed using the NIH Stroke Scale (NIHSS) and the Modified Rankin Scale (mRS), respectively.</p> Results <p>The prevalence of hypertension, diabetes, dyslipidemia, ischemic heart disease, and prior stroke history was significantly higher in patients than controls (<i>P</i> &lt; 0.001). The AA genotype of rs2241767 was the most frequent in both groups. No significant differences were found in genotype distribution concerning clinical parameters, vascular risk factors, stroke subtypes, or degree of carotid artery stenosis (<i>P</i> &gt; 0.05).</p> Conclusions <p>This study investigated the association between the ADIPOQ rs2241767 polymorphism and IS risk in the Eastern Iranian population. While no significant association was observed, the findings underscore the importance of considering population-specific genetic factors in stroke susceptibility. Enhancing IS risk prediction and prevention may benefit from future research by integrating genetic, clinical, and environmental data to develop personalized intervention strategies.</p>

Association of adiponectin rs2241767 gene polymorphism with cerebral ischemic stroke risk in the population of Eastern Iran
