Evaluating the utility of anti-human IgA, IgG, IgM, Kappa, Lambda-FITC (conjugate) cocktail antibody in transplant renal pathology

In this study, we evaluated the utility of polyclonal rabbit anti-human IgA, IgG, IgM, Kappa, Lambda - FITC antibody cocktail (CTA) in identifying immune complex deposits in conjunction with C3 & C1q antibodies in consecutive transplant renal biopsies. A total of 154 transplant renal biopsies were studied, and we found that 16 patients had immune complex-mediated diseases, which included thirteen IgA Nephropathy (IgAN), two IgA dominant Post-infectious glomerulonephritis (IgA-PIGN) and a single C3 glomerulonephritis (C3GN) patient. CTA was positive in all the patients of IgAN & IgA-PIGN and negative in C3GN and the remaining 138 patients. As the primary screening panel uses a conjunction of CTA with C3 & C1q, C3 showed positivity in the C3GN patient. Hence, specificity, sensitivity, positive and negative predictive value in diagnosing the immune-complex mediated disease is 100% in our study when CTA is used in conjunction with C3 & C1q antibody. Hence, we propose that CTA, in conjunction with C3 & C1q antibodies, is a cost-effective and time-saving screening tool for identifying immune complex deposits in conjunction with C3 and C1q antibodies in transplant renal biopsies.
