Symptomatic adrenal endothelial cyst in a young female

Adrenal cystic lesions are uncommon and largely asymptomatic. Adrenal cystic lesions are divided into four types: pseudocysts, endothelial cysts, epithelial cysts, and parasite cysts. We discuss the case of a woman in early 30 s who arrived with right lumbar discomfort, nausea, and vomiting intermittently for one month. Radiological evaluation identified a cystic lesion in the right adrenal gland, prompting a transperitoneal laparoscopic adrenalectomy. The patient recovered well postoperatively and was discharged in stable condition with minimal discomfort managed by oral analgesics. At follow-up, the patient remained asymptomatic. Histopathological analysis confirmed the lesion as an endothelial cyst, characterized by a fibrous wall lined with flattened endothelial cells and no evidence of malignancy.
