<p>Acute ischemic stroke with multiple large vessel occlusions was usually noted in unilateral internal carotid artery region or vertebrobasilar artery region, whereas some rare cases could present multiple circulatory system vessel occlusions. For patients with multiple circulatory system vessels occlusions, there were some challenges during endovascular thrombectomy, such as acute occlusion or chronic occlusion, the first-line target vessel and thrombectomy strategy. We reported two cases with multiple circulatory system vessels occlusions, with unilateral internal carotid artery chronic occlusion, treated with endovascular thrombectomy crossing communicating artery. The two cases indicated that stent-retriever thrombectomy crossing communicating artery was technically feasible and safety.</p>
