Objective <p>To investigate how nutritional status and body composition influence the quality of life in multiple myeloma patients over time, identifying nutritional parameters that may predict early deterioration.</p> Design and participants <p>This observational and prospective study recruited 47 patients (63.6% male) over 12 months at La Fe University hospital (Valencia, Spain). Follow-up was 12 months. Patients were stratified into asymptomatic and symptomatic groups, to describe the evolution.</p> Results <p>Symptomatic patients experienced significant declines in body composition parameters, particularly fat and lean mass, correlating with worsening quality of life scores. Notably, waist-hip ratio, grip strength, and fat-free mass index emerged as significant predictors of deterioration (p-value &lt; 0,001 and 0,02, respectively). Asymptomatic patients maintained relatively stable. The study highlighted the importance of using assessment tools that consider body composition rather than just total weight.</p> Conclusions <p>Nutritional status and body composition significantly impact the quality of life of these patients, with worsening scores. Early identification of muscle mass deterioration or sarcopenia could improve quality of life and long-term clinical outcomes. Future multicenter studies with larger samples and longer follow-up periods are needed to validate these findings and develop evidence-based clinical guidelines for the nutritional management of these patients.</p>
