Objective <p>Numerous studies have indicated that adherence to the Dietary Approach to Stop Hypertension (DASH) diet prevents blood pressure and cardiometabolic risks. We assessed the adherence of adults in Addis Ababa, Ethiopia, to a DASH-style diet and its association with the risk of hypertension and adiposity.</p> Design <p>We used cross-sectional data from the Surveillance of Non-Communicable Diseases in Addis Ababa (SuNCD-AA) study. The participants underwent standardized blood pressure and anthropometric measurements, including body fat percentage (%BF). The DASH score was calculated based on eight food groups constructed using a food frequency questionnaire. The average DASH score was 22.5 (95%CI:22.2–22.8), higher in women (23.7, 95%CI:23.3–24.2) compared to men (21.2, 95%CI:20.7–21.5). The highest DASH scores were observed in individuals with normal adiposity indices, non-alcohol drinkers, and 18–24 and 55–64 age groups (<i>p</i> &lt; 0.001). After controlling for confounders, individuals with the highest adherence to the DASH-style diet had 72% (<i>P</i>-trend &lt; 0.001) lower odds of hypertension compared to the lowest. Furthermore, participants with greater DASH adherence were 15%, 62%, and 82% less likely to have higher %BF, waist circumference, and waist-to-hip ratio, respectively (<i>P</i>-trend &lt; 0.001).</p> Conclusions <p>Adherence to the DASH-style diet was moderate. We found that adherence to the DASH-style diet reduced the risk of hypertension and adiposity. This healthy dietary pattern should be adopted and tailored to public health policies and clinical practice in the future.</p>
