Background <p><i>Spirulina platensis</i>, a cyanobacterium rich in bioactive compounds, has been associated with favorable effects on lipid metabolism and body composition. This study aimed to evaluate the effects of daily spirulina supplementation on anthropometric parameters, serum lipids, renal biomarkers, and dietary intake in healthy adults.</p> Methods <p>In a 12-week randomized, double-blind, placebo-controlled trial, 64 healthy participants received either spirulina (2&#xa0;g/day) or a placebo. Anthropometric indices, serum biochemical parameters, and dietary intake were assessed at baseline and post-intervention. Statisti€cal comparisons within and between groups were conducted using paired and independent-sample <i>t</i>-tests.</p> Results <p>Both groups showed significant reductions in body weight and BMI. However, spirulina supplementation was associated with a significantly greater preservation of lean mass and a 1.44% reduction in body fat (<i>p</i> = 0.041). Notably, soft lean mass in the arms and trunk increased significantly in the spirulina group (<i>p</i> &lt; 0.01). Improvements in the lipid profile were observed only in the spirulina group, with significant reductions in total cholesterol (-16.00&#xa0;mg/dL, <i>p</i> = 0.001), LDL-C (-16.97&#xa0;mg/dL, <i>p</i> &lt; 0.001), and the LDL-C/HDL-C ratio (-0.35, <i>p</i> = 0.001). No significant changes in renal biomarkers or HDL-C were detected. Dietary intake reductions were comparable between the groups, suggesting that the observed benefits were attributable to spirulina supplementation.</p> Conclusion <p>Daily supplementation with 2&#xa0;g of spirulina for 12&#xa0;weeks resulted in significant improvements in lipid profile, body composition, and lean mass preservation in healthy adults without adversely affecting renal function. These findings support the potential role of spirulina as a functional food for promoting cardiometabolic health.</p>
