Purpose <p>Growing evidence shows the potential health benefits of pigmented rice. However, the definite advantages remain uncertain. Thus, we performed a systematic review and meta-analysis to examine the effect of pigmented rice consumption on metabolic parameters (PROSPERO registration: CRD42024581032). This research was supported by the Program Management Unit for Competitiveness, Thailand.</p> Methods <p>Randomized controlled trials (RCTs) were searched from PubMed, Scopus, Embase, EBSCOhost, ProQuest, OpenGrey, Thai Digital Collection, and Thai Journal Online up to Feb 26, 2025. Cochrane risk-of-bias tool 2 was used to assess the risk of bias. Screening, selection, and data extraction were performed by two independent reviewers. A random effect model was used for meta-analysis. The certainty of evidence was assessed using GRADE.</p> Results <p>Sixteen RCTs were included and classified as acute and chronic studies. Acute studies demonstrated reductions in postprandial glucose and insulin levels. Food products incorporating pigmented rice (pudding, bread, and cereals) exhibited low to moderate glycemic index, indicating favorable glycemic profiles. The chronic studies revealed no significant effects on fasting blood sugar levels. Subgroup analysis demonstrated significant reductions in total cholesterol (MD: - 10.44, 95%CI: - 17.01, - 3.88, <i>p</i> &lt; 0.05) and low-density lipoprotein (MD: - 6.82, 95%CI: - 9.48, - 4.15, <i>p</i> &lt; 0.05), when consumed as food-based products. No significant changes were observed for triglycerides, high-density lipoprotein, body weight, or body mass index.</p> Conclusion <p>Pigmented rice offers potential benefits for glycemic and lipid control. However, the evidence remains inconclusive due to the limited number of studies and high heterogeneity. Future RCTs are needed to confirm its metabolic health benefits.</p>
