Association of life’s essential 8 with depressive symptoms in US cardiovascular disease survivors: a cross-sectional analysis from NHANES 2007–2018

In recent years, Life’s Essential 8 (LE8) has been increasingly applied as an indicator to assess cardiovascular health condition. Depression is common in patients with cardiovascular diseases (CVD). This study aimed to explore the relationship between LE8 and depressive symptoms in patients with CVD.
MethodsThe present cross-sectional study was based on the National Health and Nutrition Examination Survey (NHANES) conducted between 2007 and 2018, including 2343 participants diagnosed with CVD. The LE8 score, derived from measurements following the American Heart Association definitions, were categorized into quartiles, and presence of clinically significant depression was defined by a Patient Health Questionnaire-9 (PHQ-9) score ≥ 10. Weighted multiple logistic regression, restricted cubic spline analysis, and subgroup analyses were applied to assess the association between LE8 score and depressive symptoms.
ResultsAmong patients with CVD, higher LE8 score correlated with lower presence of depressive symptoms; each 1-point increase was associated a 4% lower risk of clinically significant depression (adjusted OR = 0.96, 95% CI: 0.95 ~ 0.98, P < 0.001). The risk was 69% lower among participants in the highest LE8 quartile (67 ~ 100) than among those in the lowest quartile (0 ~ 48) (adjusted OR = 0.31, 95% CI: 0.24 ~ 0.40, P < 0 0.001) after adjusting for age, sex, race, marital status, income, education levels, and alcohol use. Weighted restricted cubic spline analysis demonstrated a linear inverse relationship between LE8 and clinically significant depression. This association persisted consistently across numerous subgroups; however, significant interactions were found for sex and income, indicating potential modifying effects.
ConclusionsA higher LE8 score was linked to a reduced likelihood of clinically significant depression in US adults with CVD. These findings suggested that maintaining optimal cardiovascular health may help prevent depressive symptoms in this specific population.
