Development and evaluation of a prevention and health promotion literacy questionnaire for schoolchildren in France

Developing health literacy, especially for school-aged children, is essential to preventing the adoption of unhealthy behaviours. Specific measurement tools can help assess needs and evaluate actions within an evidence-based approach. This study aimed to develop a questionnaire to assess health literacy, with a focus on prevention and health promotion, and to evaluate its psychometric properties among French schoolchildren aged 8–11.
MethodsWe developed a prevention and health promotion literacy questionnaire following three steps: (i) item selection from eight prevention and health promotion domains (attitudes towards health, nutrition, physical activity, addictions, sun exposure, sleep, screen use, and safety), each organized into four parts according to the dimensions of the Capability, Opportunity, Motivation, Behaviour (COM-B) model, and content validity, (ii) pre-testing and refinement of the questionnaire, and (iii) assessment of psychometric properties and item reduction after administration to a sample of children in 30 schools.
ResultsA total of 699 children completed the questionnaire (mean age 9.6 ± 0.5 years; 51.1% girls). Dimensional analysis yielded a 29-item questionnaire with a three-factor structure: health-related motivation (dim. 1), screen use behaviours (dim. 2), and multi-thematic health knowledge dimension (dim. 3). The questionnaire demonstrated good acceptability and satisfactory reliability (Cronbach’s alpha = 0.77 for the overall questionnaire, 0.75 for dim. 1, 0.71 for dim. 2, and 0.55 for dim. 3).
ConclusionsThe questionnaire developed in this study enables an assessment of health literacy from a prevention and health promotion perspective in French school children. It offers a practical resource to identify needs and guide health promotion actions in schools.
