Background <p>Despite the availability of an effective vaccine, yellow fever (YF) remains a major public health concern in Nigeria. This study assessed YF knowledge, risk, and vaccination status among residents of selected Local Government Areas (LGAs) in Ondo State to inform targeted interventions.</p> Methods <p>A cross-sectional survey was conducted from July to August 2024 across six LGAs in Ondo State, including four with confirmed YF cases and two without. Using a three-stage sampling method, 294 adults aged ≥ 18 years were engaged. Data on socio-demographics, YF knowledge, and risk factors were collected using an interviewer-administered, semi-structured questionnaire. Descriptive statistics, chi-square tests, and logistic regression analyses were performed.</p> Results <p>Participants had a mean age of 38.8 ± 14.7 years, and 185 (62.9%) were female. Although 199 (67.7%) had heard of YF, only 46 (23.1%) demonstrated good knowledge, while 99 (33.7%) reported prior vaccination. High YF risk was identified in 105 (35.7%) respondents. LGAs with reported YF cases had higher odds of YF risk and vaccination but lower odds of having good YF knowledge (<i>p</i> &lt; 0.05). Good knowledge was associated with receipt of YF health education (aOR: 5.19; 95% CI 2.30–12.32) and residence in Akoko North-West LGA (aOR: 4.81; 95% CI 1.32–19.17). Factors associated with high YF risk included agriculture-related occupations (aOR: 5.67; 95% CI 2.98–11.16) and residence in Ondo East LGA (aOR: 6.22; 95% CI 2.04–20.31), Akoko South-West (aOR: 7.39; 95% CI 2.69–22.10), Ifedore (aOR: 3.01; 95% CI 1.08–9.00), and Akoko North-West LGAs (aOR: 4.00; 95% CI 1.31–12.91).</p> Conclusion <p>Despite moderate awareness, YF knowledge remains poor while the risk of contracting it is high in some parts of Ondo State. Targeted health education, improved vaccination coverage, and integrated vector control strategies are needed in high-risk LGAs.</p>
