Introduction <p>Considering the accelerating global shift towards digital transformation, health services have become more accessible and efficient, with significant potential to improve quality of care and strengthen health system responsiveness. Yemen remains one of the most challenged countries in this regard. Ongoing conflict for over a decade has severely disrupted the health system, resulting in the operational breakdown of nearly half of the country’s health facilities and a marked inability to provide continuity of care, particularly to vulnerable populations. One of the most pressing issues is the health system’s limited capacity to meet the needs of people living with noncommunicable diseases (NCDs), who require sustained and integrated health services and medications. Insulin which is a life-saving medicine for diabetes remains scarce in Yemen due to the supply chain disruptions and instability. This ongoing situation can cause serious deterioration in patients’ health such severe complications like kidney failure which can leads to death. What is even more concerning is the absence of a reliable health information system that could provide accurate data on NCD prevalence or support evidence-based decision-making, furthermore, there is no national database that can be relied upon to assess specific needs or to support the development of strategic plans that prioritize interventions based on population health requirements. Accordingly, this paper presents a set of digital solutions and platform models that have proven effective in similar contexts.</p> Methods <p>A comprehensive literature review was conducted across multiple reputable academic databases. Evidence was analyzed via narrative synthesis, coding findings into technical, organizational, and contextual themes, and mapped to established frameworks: the WHO Global Monitoring Framework for NCDs, the WHO STEPwise (STEPS) approach to risk-factor surveillance, and pragmatic Health Information system (HIS) evaluation criteria (functional, technical, contextual). This framework-based synthesis was then contrasted with Yemen’s constraints to derive an implementation model. A structured literature review was conducted across four databases, and 45 studies met the inclusion criteria. The selection process followed PRISMA 2020 guidelines.</p> Results <p>With earlier experiencing DHIS2 in other health programs such nutrition and EPI,using DHIS2 (as a platform with a database and analytics tools) farmwork would improve the potential in monitoring NCD in Yemen, by offering an open-source, flexible, and reliable solution equipped with advanced data analysis tools. By integrating DHIS2 into the health system, Yemen’s healthcare sector could enhance the management of NCD and improve overall healthcare delivery. This approach presents an optimal solution for reducing the burden of NCDs and promoting better health outcomes for patients by suggesting a conceptual DHIS2 framework. requires an effective response, including early detection.</p> Conclusion <p>In this context, digital transformation into fragile, conflict-affected, and vulnerable (FCV) settings, as emphasized by the World Health Organization, presents a valuable opportunity to enhance health service delivery. Adaptable digital health platforms such DHIS2 which a free, open-source platform can improve the availability, timeliness, and use of NCD indicators through offline capture, incremental scale-up, and reuse of national infrastructure and workflows. While data completeness will continue to reflect service interruptions, the proposed model offers a realistic path to better program visibility and data-informed decisions, which are prerequisites for improved patient outcomes</p>
