Drivers of electronic pillboxes adoption and HIV treatment in Africa: a scoping review

This study aimed to identify the factors influencing the use of electronic pillboxes in Africa and to outline their implications for practice and policy.
MethodsA scoping review was conducted to identify and evaluate the factors driving the use of electronic pillboxes in Africa. Searches were initially run between September 11–20, 2023, and then rerun on February 19, 2025, in PubMed (NIH.gov), CINAHL (EBSCOhost), APA PsycINFO (Ovid), and Cochrane Central Register of Controlled Trials (Ovid) and Google Scholar. PRISMA for scoping reviews methodology was used. The study was completed on March 20, 2025.
ResultsElectronic searches yielded 1,155 articles, of which 85 met the inclusion criteria. Research projects were the primary driver of device use, particularly for monitoring and measuring adherence, as documented in 50 (59%) articles. Reported success of device use was noted in 42 (49%) articles. The device was deemed unsuccessful in 6(7%) articles, while 10(12%) articles found it to have no significant impact. HIV was the most frequently studied condition, examined in 56 (66%) articles, followed by tuberculosis in 19 (22%). Other conditions included hypertension and depression. Barriers to device adoption included concerns about patient confidentiality, connectivity issues, and stigma.
ConclusionThe adoption of electronic pillboxes in Africa is largely driven by research on HIV treatment adherence. To maximize their impact, it is essential to develop policies that promote widespread integration and societal acceptance. Moreover, their use should be expanded to facilitate medication adherence across a range of other chronic health conditions.
