Background and aims <p>Diabetes, as one of the most prevalent chronic diseases worldwide, can lead to severe complications if left unmanaged within healthcare systems. This study aimed to translate the Patient Evaluation of Diabetes Care Quality Scale into Persian and evaluate its psychometric properties.</p> Methods <p>This methodological study was conducted from September to October 2024, involving 286 diabetic patients recruited from urban comprehensive health centers in Qazvin, Iran. Psychometric properties of this scale including face, content and construct validity (using confirmatory factor analysis (CFA) were assessed. CFA was conducted using Amos v.24 software.</p> Results <p>Participants had a mean (SD) age of 57.71 (11.60) years and a diabetes duration of 10.07 (7.30) years. There was negligible floor (0.5%) and ceiling (4.5%) effects for the Diabetes Service Quality Questionnaire total score. With Cronbach’s alpha of 0.95, corrected item-total correlation between 0.59 and 0.89 except for item 1 (which was deleted due to item-total correlation below 0,4 and factor loading less than 0.5 in primary CFA analysis), item level psychometric properties were acceptable. The results of the CFA showed excellent fit (CMIN/DF = 2.536, PClose = 0.077, CFI = 0.993, TLI = 0.984, NFI = 0.988, RMSEA = 0.064, SRMSR = 0.023, GFI = 0.954, and AGFI = 0.897). The CR (= 0.96) was greater than 0.70, indicating good reliability. The AVE = 0.656, MSV = 0.001 showed good convergent and divergent validity.</p> Conclusion <p>The 13-item Patient Evaluation of Diabetes Care Quality Scale demonstrated robust psychometric properties in the Iranian population. It serves as a brief, valid tool for assessing diabetic care quality and identifying areas for improvement in healthcare services.</p>
