How a hospital-based school program can support children with cystic fibrosis

For children and adolescents with cystic fibrosis (CF), hospital encounters provide an opportunity to positively intervene when school and health become conflicting priorities. This study describes the services provided by a Hospital-Based School Programs (HBSP) at a children’s hospital in the United States of America (USA) for pediatric CF patients to address educational needs.
Subjects and methodsData were collected from 105 patients with 178 encounters for the 2021–2022 and 2022–2023 academic years. Most patients had one (59.0%) followed by two (19.7%) HBSP encounters, occurring across inpatient and outpatient settings. Hospitalized patients most often received care with the pulmonary service line (N = 67, 37.6%). Length of inpatient stay ranged from 1 to 374 days (Median = 13; Interquartile Range = 10).
ResultsAt first HBSP encounter in the period, 68% of patients had formalized school supports in place, either an Individualized Education Program (21.9%), which may provide intervention services or modifications to the curriculum or Sect. 504 Accommodation Plans (41.9%), which provide accommodations to ensure that students with disabilities can access the curriculum. An additional 31 patients received recommendations for formalized school supports from the HBSP. The HBSP provided 562 school consultations across 177 encounters and direct instruction was provided during 52 of the 76 inpatient encounters, averaging 5.35 (SD = 3.43) direct instruction sessions. Other services provided included recommendations for school supports at home due to medical needs, formalized information sharing and other communication types on patient’s medical status and needs. The HBSP also provided services after discharge 51 times via school meetings or consultations.
ConclusionCompared to national peers, school supports used by this cohort on entry to the hospital and post-reintegration support were higher. This highlights the potential value of HBSPs in supporting patients with CF.
