Background <p>Trans people in Mexico City encounter stigma and structural violence throughout social, medical, and legal transitions, leading to poorer health and secondary disabilities. Yet these processes are not sufficiently documented, and community-based recommendations are rare. Methods: Multisite ethnographic study (Sept 2021 May 2024) with participant/non-participant observation and 20 semi-structured interviews (18 trans participants, two providers). Results: Discrimination, administrative hurdles, and inaccessible services drive (self-)exclusion from care. Conclusion: Propose inclusive, respectful, and empathetic care models coupled with community-led training for health professionals.</p>
