Background <p>Population aging has led to an increase in chronic conditions, highlighting the need for new healthcare models. Digital tools can increase accessibility and promote universal healthcare coverage. This study aimed to identify facilitators, barriers, and recommendations for implementing a digital-based case management (CM) program.</p> Methods <p>A qualitative descriptive design was used, employing the eDelphi method to gather insights from stakeholders. The process involved two rounds of data collection and a third round of consensus assessment, with responses processed and analysed in Excel.</p> Results <p>This study involved 12 experts, mostly females, aged 41–50 years, with diverse health sector experience. Key concerns included digital literacy and insufficient investment in digital infrastructure, while the importance of digital training was emphasised. Consensus was achieved, with over 70% agreement among participants.</p> Conclusions <p>The findings offer a clear framework for developing a CM program using digital tools, aligning with literature and current projects.</p>

Background
