Sense of coherence is associated with daily free sugar consumption and oral health-related quality of life in 15-year-old school children attending government schools in Kandy district, Sri Lanka: a cross-sectional analytical study

Sense of Coherence (SOC) has been recognized as a significant psychosocial determinant of health. Evidence related to the relationship between SOC and oral health among adolescents and children are scarce. This study aimed to determine the associations between the SOC and oral health based on oral health status, oral health-related behaviours, and oral health-related quality of life in 15-year-old school children attending government schools in Kandy district, Sri Lanka.
MethodsA cross-sectional study was conducted among 700, 15-year-olds attending government schools in Kandy district, Sri Lanka. Two-stage cluster sampling technique combined with the probability proportionate to size sampling method was used to select the participants. SOC was measured using a validated Sinhala version of the adolescents’ SOC questionnaire. Oral health behaviours and oral health-related quality of life were assessed using a self-reported questionnaire and a validated Sinhalese version of the 16-item Child Perception Questionnaire 11–14 (CPQ) respectively. Oral health status was determined based on the caries experience, bleeding on probing, and periodontal pocket depth. Independent associations between SOC and oral health indicators were determined using binary logistic regression models.
ResultsParticipants with a low SOC were more likely to consume sugary foods daily (OR = 0.93, 95% CI: 0.88–0.97) compared to those with a high SOC when adjusted for confounding factors. SOC was associated with both the total CPQ score and domain-specific CPQ scores, with higher SOC scores indicating better oral health-related quality of life. SOC scores were not associated with oral health status.
ConclusionSOC was associated with daily consumption of foods high in free sugars and good oral health-related quality of life. Therefore, interventions based on SOC could be used to enhance oral health behaviours and oral health-related quality of life of adolescents in Sri Lanka.
